Select one patient/client with whom you will be working during your Internship. Keep notes of the patient’s/client’s progress during your intervention and submit a written Patient/Client Evaluation report to the University Supervisor at the end of your Internship using the following report format:

1. Patient’s/Client’s Longitudinal History (Milestones)
2. Patient’s/Client’s Current Medical, Psychological, Educational Background
3. Patient’s/Client’s Needs Assessment (explain assessment procedure used and include the following, if applicable)
   1. Task Analysis (observation)
   2. Diagnostic Test
   3. Conference with Patient/Client
4. Terminal Goals and Behavioral Objectives of Program for Client.
5. Anecdotal Record of Client’s Program
   1. Physical and/or Motor Objectives
   2. Social Objectives
   3. Reinforcement Techniques
6. Summary of Patient’s/Client’s Progress During Period of Intervention
