Section One
• Describes Amina Jr and her circumstances. This may include age, social circumstances, where does she live, family, occupation , ethnicity etc.
• Considers Amina Jr’s wellbeing including physical, mental and emotional.
• Considers the social determinants of health that could contribute to her life and outcome (socio-economic, environment, culture, education, lifestyle). Then choose one or two and consider them in some detail in relation to risk factors and lifestyle.
• Considers how the different determinants impact each other and link together.
• Uses relevant statistics and data to illustrate how the social determinants can impact upon health.
• Explores health literacy. Include a definition. Consider impacts of poor literacy on an individual and society. Considers how health literacy impacts upon other health determinants. Explains how those with poor literacy levels can be supported to improve health outcomes.
• Considers ACE’s and how they relate to Amina Jr’s situation.
• Considers the steps that Amina Jr is taking to improve her situation and that of her young daughter. Relates this to empowerment.
• Includes relevant definitions of key terms including social determinants of health, health literacy.
• Considers how the topic relates to government / local policy directives. E.g. NICE guidelines, local / regional strategies.
• Includes some relevant data / statistics to illustrate the local environment. For example the prevalence of living in temporary accommodation in Leeds.

Section Two
• Describes the role of the nurse in relation to behaviour change drawing on the information from section one.
• Describes and discusses behaviour change models . Include pros and cons of each.
• Discusses why it is important for nurses to understand behaviour change. Include the potential consequences on the individual, community and society.
• Discusses what facilitates or inhibits behaviour change? Considers social determinants of health , health literacy, cycle of change)
• Includes some relevant data / statistics to your points.
• Considers community initiatives that help individuals to help themselves. i.e. Dadz
• Relates information to government / local policy .
• Considers the role of the nurse in promoting behaviour change. Considers what strategies can be used by the nurse to tackle the issue, what skills may the nurse need for example
o Individual/population focus
o Brief intervention, motivational interviewing,
o Communication, empathy, empowering, local knowledge
o Advocate for service redesign
o Nurses role as an educator including pros and cons
• Considers any ethical considerations that the nurse needs to be aware of, from an individual or a larger perspective i.e. NMC
