injuries), medications, hospitalizations, and disabilities. Ques-
tions may include the following:
• What is your current, overall health?
• Have you ever had a serious medical illness or injury?
• Have you ever been hospitalized for a medical
problem?
• Are you taking any medications related to a medical
problem?
